Pest control service report and certificate of treatment: what was found, what was used, where, and what the client must do.
A two-page service report (findings by area, products with active ingredient and EPA approval, safety advice, recommendations, follow-up) and a one-page certificate of treatment landlords, property managers and food businesses ask for. PDF and editable Word. Free, no email required.

What should a pest control report include, and do pest controllers give a certificate in NZ?
The report records the inspection findings by area with the level of activity and the conditions that attract pests; every product applied with its trade name, active ingredient, EPA approval number, method, location and quantity; the safety advice given (re-entry times, bait station locations, children and pets); the client’s own actions; and the next visit. That is both what the client needs and the record a business must keep under the Health and Safety at Work (Hazardous Substances) Regulations 2017, which also require an inventory, current safety data sheets and certified handlers for certain substances. A certificate is not required by law, but it is routinely asked for by landlords, property managers, food businesses and buyers, and the one in this pack states the pests, areas, products, outcome and any warranty on one page. Across 1,876 publicly posted reviews of NZ pest control businesses, clear explanation is praised in 41% of positive reviews, the highest of the three property maintenance trades, and a follow-up after the visit in 5.5%, also the highest. A report handed over on the day is both.
The job this document is doing.
- —The client asks what was sprayed near the kids’ room and the technician cannot say from memory
- —A food business is audited and has no record of the last treatment
- —Bait stations are placed and never mapped, so the follow-up visit cannot find them
- —The ‘warranty’ was verbal and means something different to each side
